People commit to recovery, reentry, or stability with real intention. The problem appears when support systems reduce structure right after that decision. Public health data shows relapse rates between 40 percent and 70 percent for people leaving treatment without ongoing support. Some studies report rates above 80 percent within one year when transitions lack structure. During these periods, accountability weakens, expectations fade, and daily stability becomes harder to maintain. Progress becomes fragile because people lose the consistent support required to continue moving forward. This transition gap is the issue Pathways is focused on fixing.
